SUMMARY                                                                                                                                                                          

 The Virtual Nurse Coordinator(VNC) serves as a key member of the Hospital at Home team, delivering comprehensive, patient-centered acute care in the home as a safe and effective alternative to traditional hospitalization. Leveraging advanced clinical judgment, critical thinking, technology, and evidence-based practice, the RN provides coordinated virtual and in-home care while serving as a patient advocate throughout the care journey.

Working collaboratively with interdisciplinary teams, the RN coordinates transitions of care, assesses patients' clinical and psychosocial needs, addresses social determinants of health, and partners with patients, caregivers, and community resources to promote successful recovery. As the primary point of contact for patients and care providers, the RN ensures seamless communication, continuity of care, patient safety, and adherence to organizational standards and clinical protocols.

This role contributes to the ongoing development and refinement of an innovative home-based care model by improving patient outcomes, reducing hospital readmissions and emergency department utilization, enhancing the patient and provider experience, and advancing the delivery of high-quality, coordinated healthcare in the home.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES (Other duties may be assigned)

  • Utilize clinical judgment and critical thinking to assess, monitor, and respond to changes in patient condition, escalating concerns appropriately.
  • Coordinate care across the interdisciplinary team to ensure seamless transitions, continuity of care, and timely interventions.
  • Serve as a patient and family advocate by promoting shared decision-making, patient education, and engagement throughout the care journey.
  • Collaborate with patients, caregivers, providers, community partners, and ancillary services to develop and implement individualized care plans.
  • Assess and address clinical, psychosocial, environmental, and social determinants of health that may impact patient outcomes.
  • Leverage virtual care technologies, remote patient monitoring, and electronic health record (EHR) systems to support safe and effective care delivery.
  • Maintain accurate, timely, and complete clinical documentation in accordance with regulatory, accreditation, and organizational requirements.
  • Establish a collaborative relationship with the patient and their family to identify needs, ensure appropriate access to resources/services, address social determinants of health, and facilitate a safe care transition.
  • Coach the patient to reach their optimum level of wellness, self-management, and functional capability.
  • Assess health learning preferences and/or barriers and provide individualized resources and tailored education via the most appropriate modality for each patient.
  • Participate in daily interdisciplinary huddles and quality improvement activities as assigned
  • Performs miscellaneous job-related duties as assigned.
  • May be required:  TB, MMR, Varicella, Hep-B and Influenza

NON-ESSENTIAL DUTIES AND RESPONSIBILITIES

QUALIFICATIONS                                                                                                                                                                 

 Education:

  • Bachelor’s degree in nursing
  • Certification/Licensure in state of residence as a licensed Registered Nurse

Preferred Qualifications:

  • Compact RN license (preferred)
  • Willingness and ability to obtain and maintain additional state RN licensure, as needed, to provide remote clinical care across multiple states

Experience:

  • Minimum of three (3) years of Registered Nurse (RN) experience in acute care, medical-surgical, emergency, home health, oncology, geriatrics, case management, care coordination, or a related clinical specialty
  • Current Basic Life Support (BLS)/CPR certification required upon hire and maintained throughout employment.

Pay range for position: $38 per hour-$51 per hour

 

PHYSICAL DEMANDS                                                                                                                                                         

 

While performing the duties of this job, team members are regularly required to sit, stand, walk, reach with hands and arms, and to talk and hear. Team members may be occasionally required to climb or balance, stoop, kneel, or crouch. Team members must occasionally lift, push and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. 

 

WORK ENVIRONMENT                                                                                                                                                      

This position is considered remote with possible travel to practice locations in other states. The noise level in the work environment is typical of an office setting.
